Video 3: Your Journey Beyond Inpatient Care

Thank you for choosing SUN Behavioral.  This video is to help you understand how SUN helps you build a plan of care after you’ve completed your inpatient stay.

You can continue to build upon the success of your inpatient recovery long after you’ve completed our program.  While you’re still inpatient with us, your therapist and discharge planner will help you build a continuing plan of care.  Once you’ve been stabilized, it’s our job to help you get back to life with the necessary supports in place.

Creating a plan for continued care is a collaborative process between you and your clinical team at SUN.  Depending on your own preferences, your clinical team will provide you with options to continue your progress with a residential program, outpatient services, therapy, counseling, or more.  Some of these services are available at SUN, while others are provided by our partners in the community.

In addition to our inpatient services, SUN provides three outpatient programs for adults.  These include a Partial Hospitalization Program (or PHP), and an Intensive Outpatient Program (or IOP).

Your clinical team will work with you to find a program that works for you and get you enrolled.  We will work to ensure no lapse in your medications or therapy.  Our goal is to ensure a smooth transition between SUN and your next healthcare provider.

Throughout the planning process, our team relies heavily on your input and preferences.  We are here to support your goals as you move forward in your journey to wellness.
